Below are some of the required camp forms you can print out, or save to your computer. Left-click to open or Right-click and select "Save link as..." to save to your desktop. All forms are in PDF format.

• Employment Application Form - If you are interested in working at Camp New Hope. Apply online or print and mail in a PDF.
• Annual Information - This is required for all programs. 
• Local Application Packet for Coles, Cumberland, Douglas and Shelby counties.
• Non-Local Application Packet for all other counties.

• Health History Form - This is required for all programs.
• Medical Authorization Form - This is required for all programs.
• Financial Aid Form - This is for campers and families who are applying for financial aid so they can attend summer camp or attend the Respite.
• Camper Code of Conduct - This is our camper expectations.
• IL Jaycees Form - This is for Jaycee Chapters that want to sponsor a camper.
• Volunteer Application Form - This is for those who wish to volunteer. 
• Employment Reference Form - This is needed if you are interested in working at Camp New Hope.
• Sponsorship Form - Please fill this out if you would like to become a sponsor.
